The short answer
For 1 adult around 30, published plans start at about €140 to €220 a month without the USA. Add the USA and the same plans cost €379 to €575.
By 60, the same mid-range plan costs €580 a month without the USA, and €1,625 with it. Age and the USA are the 2 biggest levers by far.

Every plan prices you by age band. On DIA-Weltpolice Plus with a €500 deductible, the monthly price goes from €220 at 25 to 34, to €580 at 60 to 64, to €1,245 at 75 and over.
The price moves up when you enter a new band. So check the price for the next band, not only today’s.

Most plans let you choose worldwide cover with or without the USA. Cigna Global offers both on every tier.
Including the USA costs 2 to 3 times more. Foyer Global Health Essential at 30 is €140 a month without the USA and €379 with it. Only pay for the USA if you live there or go often.

A deductible is what you pay yourself each year before the plan pays. On DIA-Weltpolice Plus, a €1,000 deductible cuts the price by €95 a month at every age. That is €1,140 a year saved for at most €1,000 of extra bills.
Outpatient cover pays for doctor visits, drugs and tests without a hospital stay. On Cigna Global it is an optional module on top of hospital cover. It is the care you use most, so leaving it out means paying those bills yourself.

Each family member is priced on their own age band. With DIA-Weltpolice Plus without the USA, 2 adults aged 35 to 44 and 2 children cost about €780 a month: €230 for each adult and €160 for each child.
Existing conditions can add to the price. BDAE adds a surcharge based on your health, and may make no offer if the risk is too high. Genki Native decides case by case, and can accept, adjust or decline.

How much does international health insurance cost?+
For 1 adult around 30, published plans run about €140 to €220 a month without the USA, and €379 to €575 with it. At 60 to 64, one mid-range plan costs €580 without the USA.
Why is cover in the USA so expensive?+
The published prices show it: adding the USA costs 2 to 3 times more at every age. Most plans let you leave the USA out if you don’t live or travel there.
Is a higher deductible worth it?+
Often, if you are healthy. On DIA-Weltpolice Plus, a €1,000 deductible saves €95 a month, or €1,140 a year, which is more than the deductible itself.
Can I get cover with a pre-existing condition?+
Often yes, for a higher price. Some insurers add a surcharge based on your health, some exclude the condition, and some decline. Disclose it, or a claim can be refused.
